Catholic Extension requires that all diocesan funding requests be made through our online system. The following funding request draft forms are available if you would like to prepare your request on a separate document before transferring the information into the online funding request form. When you are ready to begin a new online funding request, click here.
Please note: funding requests submitted by any other means will not be considered for funding.

Outcome Evaluation Report Draft Form
Recipients of Catholic Extension funding are required to complete and submit an Outcome Evaluation Report online within one year of the reception date. The following outcome evaluation report draft form is available if you would like to prepare your report on a separate document before submitting it online. When the draft is complete, please click the blue “Access Saved Funding Requests” button to log in to your account and submit the report.
